By signing this form, you are choosing to participate in a tandem or solo skydive operated by our staff. You recognize that jumping from an aircraft and navigating back to the ground under a canopy involves significant physical risks and that you are choosing to engage in this activity of your own free will.

You understand that skydiving is an inherently dangerous activity that carries the risk of serious injury, permanent disability, or death. These dangers include, but are not limited to, potential equipment malfunction during freefall or deployment, unpredictable weather conditions, turbulent air, and accidents occurring during takeoff, flight, or landing. You accept full responsibility for these risks and agree that your decision to jump is based on your own assessment of these hazards, regardless of any information provided by the instructors.

Release of Liability

In consideration for being permitted to skydive, you hereby release, waive, and discharge our company, its instructors, aircraft pilots, and parachute riggers from any and all liability for personal injury, property damage, or wrongful death resulting from the ordinary negligence of the operator. This release applies to all phases of the skydiving experience, including ground training, the ascent, the jump itself, and the final landing, provided that such harm arises from the risks inherent to the sport.
