I consent to receive any medical treatment deemed advisable in the event of an injury, accident, or illness during these activities. I further acknowledge that any participant under the age of 18 must have this document signed by a parent or legal guardian to be eligible to participate.
I HAVE READ THIS ASSUMPTION-OF-RISK, WAIVER, AND RELEASE AGREEMENT IN ITS ENTIRETY AND I CONSENT TO MEDICAL TREATMENT AS STATED ABOVE. I FULLY UNDERSTAND ITS TERMS AND RECOGNIZE THAT BY SIGNING THIS DOCUMENT, I AM GIVING UP SUBSTANTIAL LEGAL RIGHTS.
