Questions about this waiver

What specific sections should I add to the Martial Arts Waiver Form to account for different training levels?
You can use the form editor to add a checkbox section where students identify their current experience level or specific modules they are enrolled in, such as beginners' grappling or advanced sparring. This helps your instructors immediately know which techniques a student is permitted to practice during class.
Should I include fields for medical conditions or emergency contacts on this martial arts release?
Yes, incorporating a section for health history and primary emergency contact details is standard for high-contact sports. Our form builder allows you to add required text fields for allergies or medical alerts so your coaching staff has immediate access to critical safety information.
Can I integrate this form into my gym's digital check-in process?
You can generate a QR code for your specific Martial Arts Waiver Form that students can scan upon entering the gym. This lets them sign the document directly on their mobile device or your front desk tablet, ensuring everyone is checked in before stepping onto the mat.
Is it possible to automate the renewal of this waiver for long-term members?
Our system tracks when each signature was collected, allowing you to filter your member list by date. You can set up automated reminders to prompt students to update their information or re-sign the form if your gym requires annual renewals for participation.
How can I personalize the appearance of the form to match my dojo's branding?
You can upload your gym's logo and select primary colors in the document settings to ensure the form feels like an extension of your studio. The interface allows you to add your specific branding to both the digital sign-up screen and any exported PDF copies.
