Dance Studio Waiver Form

By enrolling in dance classes at this studio, you are choosing to participate in physically demanding movement activities, including choreography, floor work, and performance training, and you acknowledge that these sessions involve inherent risks of physical exertion and accidental injury.

You understand that dance training involves significant physical contact with studio surfaces, participation in high-impact movements, and the potential for collisions or falls. By signing this document, you confirm that you are aware of these inherent risks, including the possibility of strains, sprains, fractures, or other bodily injuries that can occur regardless of the level of supervision or the condition of the studio floor, and you voluntarily assume full responsibility for choosing to engage in these movements.

In exchange for permission to use our studio facilities and receive instruction, you hereby release, waive, and discharge this dance studio, its instructors, and its agents from any and all liability, claims, or demands arising out of any loss, damage, or injury, including death, that may be sustained by you while participating in classes or on the premises, including those caused by the ordinary negligence of the studio or its staff.

I confirm that I have read this document in its entirety, that I fully understand its terms, and that I am signing it freely and voluntarily. I further certify that I am at least 18 years of age, or that I am the parent or legal guardian of the participant and have the authority to sign on their behalf.
