Cryotherapy Waiver Form

You are voluntarily choosing to participate in a whole-body cryotherapy session provided at this facility. By proceeding, you acknowledge that you have been informed of the nature of this cold-exposure treatment and understand that your participation involves inherent risks to your health and physical wellbeing.

I understand that cryotherapy involves exposure to extreme sub-zero temperatures, which carries specific risks including, but not limited to, frostbite, skin irritation, cold burns, dizziness, headaches, and potential respiratory distress. I acknowledge that these risks are intrinsic to the operation of the cryotherapy chamber. I have reviewed the contraindication list and confirm that I have no underlying medical conditions, such as hypertension or cardiovascular issues, that would make this exposure unsafe for me. I freely accept and assume all such risks related to my session and any potential physical consequences resulting therefrom.

In consideration for being permitted to use the cryotherapy equipment, I hereby release, waive, and discharge the facility, its owners, employees, and agents from any and all liability, claims, or demands arising out of any loss, damage, or injury, including death, that may be sustained by me while on the premises or during the session. This release applies to any claims resulting from the ordinary negligence of the facility or its staff. I agree that I am solely responsible for my own physical condition and for consulting with a physician prior to exposure if I have concerns regarding my suitability for this activity.

I certify that I have read this document in its entirety, fully understand its terms, and am signing it freely and voluntarily. I confirm that I am at least 18 years of age and possess the legal capacity to enter into this binding agreement regarding my participation in cryotherapy.
